GDPR

New legislation called General Data Protection Regulation (GDPR) comes into effect on May 25th 2018. This legislation changes the way businesses are alowed to collect, store and use personal data.

To comply with these changes and to maintain a duty of care to my class participants, I need to keep a record of everyone's name,medical details and a contact number for each individual. This information allows me to modify and adapt class content to suit everyone and to be able to contact each individual if I need to cancel a class.

All personal information is collected and stored for my sole use and will not be shared with any third parties. This information will be collected via a PAR-Q form in each class.

Here is a summary of the information I collect, why it is required and how it will be stored:

Your Name: To identify each individual and this is a prerequisite for insurance purposes. PAR-Q forms are stored in a locked cabinet with myself (Karen Bent) as the sole keyholder.

Contact numbers:To allow me (Karen Bent) to contact you if a class is cancelled. All phone numbers are stored on my personal mobile which is password protected.

Medical & Fitness history: To enable me to offer appropriate guidance in class in terms of exercise modifications and adaptations. PAR-Q forms are stored in a locked cabinet with myself (Karen Bent) as the sole keyholder.

All screening forms need to be signed and dated, both as a prerequisite for insurance purposes and to remain compliant with current legislation.

You have the right to access, restrict or withdraw your personal information at any time.

PAR-Q forms distributed in all classes EXPLICITLY request your permission to record, use and store the data collected. Under no circumstances will this information be shared with any third parties.
